Your Initial Visit
Paperwork
Before your first visit we do require some information about you and your health. There are 3 possible ways you can accomplish this.
1. You may submit the Registration and Medical History information online. These forms will be automatically submitted to us after you complete the information online. This is the most popular method as it saves time on the day of your visit and allows you to make the most out of your consultation time with us.
2. You may download and print paper copies of the registration and medical history forms and complete them. You can either bring the completed forms with you on your visit or mail to us ahead of time.
3. You may use the traditional option of completing the registration and medical history information in our office on the day of your visit. If you choose this option please plan to arrive 15 minutes early to allow sufficient time for this.

Parking
There is underground parking and validation is provided for the duration of your office visits and procedures. When you enter the parking garage, please follow the signs marked "West Building" and enter through the green elevator bank, up to the first floor. As the parking garage gets very busy during the day, please proceed directly to the lower levels (P3 and P4) for open parking spaces.
Consultation fee
There is a $75 fee for cosmetic consultations with Dr. Naficy or Dr. Sattler. This fee is fully credited toward any surgical procedure you choose to have. For facial plastic surgery and rhinoplasty you also have the option of requesting a preliminary cosmetic consultation with Julie Legman, Dr. Naficy's patient care coordinator, which will be free of charge. Consultations with your provider for all non-surgical procedures are complimentary as well. When scheduling an appointment with Dr. Naficy or Dr. Sattler, you may give us your Credit Card information when you call to schedule a consultation.
Cancellation policy
We respect the value of your time and ask the same of you. Therefore, we request the courtesy of a 48 hour advanced notice for rescheduling or canceling your appointment. If this request is not honored, we reserve the right to ask for pre-payment of your fee by credit card prior to rescheduling another appointment for you. Consultations with Dr. Naficy or Dr. Sattler require pre-payment of the $75 consultation fee, and this payment is fully refundable with a 48 hour notice of rescheduling or cancellation.
Privacy
The privacy of your personal and medical information will be fully maintained by our office. Also, during the course of your surgical procedure and subsequent visits, we will arrange for your entry and exit through a private door to the office.
