Virtual Consultation
We understand that you may have a busy schedule and may prefer to get a general idea of whether you are a good candidate for the procedure(s) you are considering. Although there is no substitute for a face to face consultation with Dr. Naficy, we have attempted to provide you with a method to determine whether you are a suitable candidate for rhinoplasty or other facial plastic surgery procedures. You may take the following steps to provide us with basic information and appropriate photographs:
1. Please complete the registration and medical history forms. These forms may be filled out, scanned and e-mailed to us, or the hard copies may be submitted by mail. Alternatively you may use the online version of these forms which will be automatically submitted to us via e-mail.
2. Please fill out one or both of the following forms and submit to us either by mail, scan & e-mail, or fax:
Use the Rhinoplasty form for nasal surgery and the Facial Plastic Surgery form for all other procedures.
3. Please get the necessary photos taken. It is important that you send us photographs of sufficient quality so that we can adequately assess your cosmetic concerns. Use a zoom lens with adequate lighting and a solid color background. Most digital cameras with a zoom lens on a macro setting should be sufficient for these pictures. Alternatively, any portrait studio should be able to take these shots for you easily and at a low cost.
Photos can be emailed to jlegman@seattleface.com or they can be mailed to the address listed below. Please label digital photos with your name (i.e. for Julia Roberts, label the photos jroberts1, jroberts2, etc.). The numbering order is not important. If you mail your printed photographs, please write your name on the back of each print.
4. The facial photos needed are: 2 side views, 2 oblique (or 3/4) views, 1 front view. For rhinoplasty procedures, you will also need a basal view, showing the bottom of the nose. See following example:

5. Mail the completed forms and photos to our office with a $75 consultation fee (Payable to Sam Naficy, MD - this payment will be applied toward your procedure if you are selected as a suitable candidate). Alternatively, you may either fax the forms or scan the forms and e-mail with your digital photos to jlegman@seattleface.com. She will contact you for your credit card information. 6. Our office will contact you within 48 hours of receiving all information via phone or email with the results of your screening consultation.
DETAILED PHOTOGRAPH INSTRUCTIONS
- Please avoid heavy make-up on the face.
- For the 5 or 6 necessary photos please use a portrait lens (90 - 105mm Macro) with adequate lighting and a solid color (preferably light blue) background
- The facial photos needed are: 2 profile (side) views, 2 oblique (or 3/4) views, 1 front view. For rhinoplasty procedures, you will also need a basal view, showing the bottom of the nose.
- Front: The front view shows the entire face and at least the upper 1/3 of the neck.
- Profile: Each profile view shows the side of the face and at least the upper 1/3 of the neck in a side view. Ideally, the head is not tilted and the gaze is directly horizontal.
- Oblique: Each oblique view shows the side of the face and at least the upper 1/3 of the neck in a 3/4 view. Ideally, the head is not tilted, the gaze is directly horizontal, and the tip of the nose is vertically aligned with the pupil of the opposite eye.
- Basal: The base view shows the nostrils and allows assessment of symmetry and a better idea of the width of the tip of the nose.
